Registered Nurse (RN) | Case Manager Location: Mission Hills, CA Agency: Cynet Health Pay: $2,682 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/23/2026 About the Position Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years of acute care hospital case management experience in care coordination and discharge planning License: Must hold a current state license; pending licenses accepted if licensed upon start date Certifications: Must have relevant certifications Must-Have: Hospital experience is mandatory Description: Start date: ASAP. Floating expectation: No. Patient ratios: 1:20. First-timers accepted: Yes; clinical will screen first-time travelers. Weekend commitment required: Yes, rotation needed to meet department needs. Experience in care coordination and discharge planning is essential. Knowledge of InterQual criteria and DRG is necessary. Experience with drug/alcohol abuse treatment options and handling patients experiencing homelessness is advantageous. Ability to organize and participate in family meetings is important. The candidate must be competent in arranging durable medical equipment and transport. Clinical documentation must be performed using computerized systems. Proficiency in Microsoft applications is required. Travelers are expected to float between facilities within a given state. Refusal to float may result in assignment termination. References: Minimum of two supervisory references covering one year of employment within the last three years in a comparable specialty/setting are needed.
