Registered Nurse (RN) | Case Manager Location: Mission Hills, CA Agency: Compunnel Healthcare Pay: $2,800 to $2,860 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position TravelNurseSource is working with Compunnel Healthcare to find a qualified Case Manager RN in Mission Hills, California, 91345! 2-year acute care Hospital Case Manager experience performing Care Coordination and Discharge planning, HOSPITAL EXPERIENCE IS A MUST • Admission Criteria (InterQual) (ED only) • Experience with Assessing patients, Care Coordination, and Discharge Planning • Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, POLST, protective services process, etc. • Ability to organize and participate in family meetings. • Participate in the development of the treatment plan. • Experience with HMOs, IPAs, etc. • Competent with arranging DME, SNF and ambulance transport.
