Key Responsibilities
Support the vision for the Care Coordination function to enhance
efficiency and referral yield
Lead a team to ensure efficient client intake processes using
technology and human interventions
Manage and develop the Care Coordination team responsible for
client-facing operations and engagement
Required Qualifications
3-5 years of experience in a Care Coordination function
2-3 years of experience leading care coordination teams in
healthcare companies
Experience with end-to-end client conversion processes using
technology and human intervention
Proficiency in Salesforce and data analytics platforms
Preferred experience in mental healthcare startups and working with
Enterprise Partners
